Summary of Employee Benefit Programs
GE Johnson provides a comprehensive benefit program to its employees. The objectives of the program are to provide:
- Support for the health and welfare of our employees;
- The opportunity to build long term financial security; and
- The opportunity for work and life balance.
The following is a summary of all benefit programs available to full-time, salaried GE Johnson employees (exempt and non-exempt, excluding craft). For benefit eligibility purposes, full-time team members are those individuals who consistently work at least 30 hours per week.
Medical Coverage
GE Johnson's medical administrator is United Healthcare. Employees are eligible for medical coverage as of the first day of employment. Highlights of the plan include:

Employees pay for a portion of the medical plan premium on a pre-tax basis as outlined in the following chart:

Dental Coverage
GE Johnson's dental administrator is Delta Dental of Colorado. Employees are eligible for dental coverage as of the first day of employment. Highlights of the plan include:
- Diagnostic and preventive procedures are covered at 100%.
- Basic procedures (e.g. fillings) are covered at 80% after a $50 per person or $150 per family calendar year deductible.
- Major procedures (e.g. caps, crowns) are covered at 50% after a $50 per person or $150 per family calendar year deductible.
- Orthodontic procedures are covered at 50% (up to a $2,000 lifetime maximum) for each covered dependent child, up to age 19.
- An annual benefit maximum per covered person of $2,000.
- Employees pay for a portion of the dental plan premium on a pre-tax basis as outlined in the following chart

401(K) Plan
The GE Johnson Construction Company 401(k) plan is designed to help you accumulate retirement income through tax-deferred savings. If you are over 21 years of age, you are eligible after completing one year of service with the company and having worked 1,000 hours during the last year. The plan entry dates are January 1, April 1, July 1 and October 1. This is how the plan works:
Employees receive a $1.00 for $1.00 company match on the first 6% of base salary contributed to the 401(k) plan. The employee is always 100% vested in the contributions you choose to defer and in the employer's matching contribution. In 2006, up to 30% of base salary can be contributed to the 401(k) plan, up to a maximum of $15,000 (or $20,000 for individuals who are 50 years or older). Employee contributions are made on a pre-tax basis.
Basic Life Insurance
GE Johnson provides salaried employees with life insurance coverage and accidental death and dismemberment (AD&D) benefits of 1.5 times your base annual salary up to $150,000 each. There is no cost to the employee for either of these benefits. AD&D coverage pays a benefit in addition to your life insurance if you die as the result of an accident. Dependent life benefits are also included.
Long Term Disability Insurance (LTD)
GE Johnson provides LTD insurance at no cost to the employee. This coverage provides monthly income benefits if you become disabled from a sickness or injury. The amount of the monthly income benefit for salaried employees equals 60% of your basic monthly income to a maximum of $10,000. LTD begins after 90 days of disability.
Short Term Disability Insurance (STD)
GE Johnson provides STD insurance at no cost to the employee. This coverage provides monthly income benefits if you become disabled from a sickness or injury.The amount of the benefit for salaried employees equals 60% of your basic monthly income to a maximum of $10,000. STD begins the eighth day after determination of disability.
Vacation Time
During the first calendar year of employment, staff employees accrue four hours of vacation for each month worked to a maximum of five days. These vacation days may be taken during the first year after satisfying a three-month waiting period. Thereafter two weeks of vacation are given each January. The vacation schedule is as follows:

Holidays
The following six (6) holidays will be observed in a typical calendar year:
- New Years Day
- Labor Day
- Memorial Day
- Thanksgiving Day
- Independence Day
- Christmas Day
Employee Assistance Program
The EAP provides resources for support and information during difficult times, as well as consultation on more day-to-day concerns. Areas of support include, but are not limited to relationship issues, substance abuse, stress, grief, financial difficulties, legal services and elder care.
Continuing Education
The Company recognizes the need for continuing education for staff employees. At least two years of continuous full-time employment is required for eligibility in this program. The curriculum must be directly related to improving individual skills and knowledge to benefit the employee's career goals within the company and attendance must not conflict with normal work hours. Attendance at seminars and the Company training classes will also be considered for approval by the immediate supervisor.

If there is any discrepancy between the terms and conditions of the various programs outlined in this summary and the plan documents, the plan documents will prevail.
